If you are paying by credit card then one of our staff members has asked you to sign our credit card authorization form.

Most customers do not need these individual forms. However, there might be a case in which a Sports & Imports Collision employee has asked for a specific form. If that’s the case, then these forms are for you! All of these forms can be downloaded and emailed back to us.

AUTHORIZATION TO WORK

This form gives Sports & Imports permission to repair your vehicle. We ask that you make sure all your personal belongings have been removed from your vehicle.

DIRECTION TO PAY SHOP

After the initial estimate, if any additional items are needed, this form directs the insurance company to send those funds straight to the shop for your repair.

If your insurance company varies from our rates, this form is your acknowledgement that it will be your responsibility to pay any differences at time of delivery.
